How do I order a death certificate from 1945-1953?

If you know the Name of the deceased and the Year of death and one of the following; Month/Day, or County, or Certificate number, you can order copies of Ohio death certificates by filling out the online order form.
Fill in the required fields, click the Request Certificate button. Then you will see above right corner: "View Bag". Select a delivery method and purchase through our online store.
