The information on Ohio death certificates is provided by the informant at time of death. The inclusion and accuracy of that information is dependent on the informant knowing and supplying the correct information.

To file a correction for Ohio death certificates from December 20, 1908 to 1970, complete an affidavit, have it notarized and mail it to:

Columbus, Ohio 43211

We will keep the affidavit on file. Please note: We cannot correct the original certificate or issue a new certificate. Also we cannot validate or certify the corrections. The State Archives sends a letter acknowledging receipt of the affidavit and stating that it will be kept on file in the State Archives.
